Rabbi David Zakut's Prayer for the Sick with Candle Lighting
Le-Refua Shlema (name and mother's name) in accordance with the words of Rabbi David Zakut—for strength, healing, and assistance from Heaven.
Recite the prayer, then light a candle and read Tehillim 139.
“Yehi ratzon Milefanekha, Ado-nai Eloheinu, ve-Elohei Avoteinu, Melekh Rachum ve-Chanun, she-bizkhut u-vekoach hadlakat ha-ner she-adlik Atah Ado-nai keneged “Ner Ado-nai—nishmat adam” (Mishlei 20:27), “Ki ner mitzvah, ve-Torah—or” (Mishlei 6:23), ve-ta'ir Nishmat (name and mother's name), ve-areini mekashèret atzmi le-Rabbi David Zakut (zt”l) ve-khivanti ke-khavanato u-khe-da'at kol ha-Tzadikim ve-ha-Chasidim, ve-areini madleka ner le-zkhut u-le-refuot (name and mother's name), she-ezkeh le-Refua shlema, refuat ha-nefesh, ve-refuat ha-guf, u-vekoach, u-ve-zkhut hadlakat ner Ado-nai ta'ir nishmat adam ve-titchazek nishmato shel kavod ha-Rav ve-ta'ir be-khol ramach (248) eivarav ve-shesah (365) gidav ve-ta'ir be-orot ha-elyonim ad Ein Sof Barukh Hu ve-ezkeh lakum mi-cholyo bari ve-shalem le-arikhut yamim ve-shanim ve-yafutzu ma'ayanotav chutzah.”
Translation:
(May it be Your will, G-d, our G-d and the G-d of our fathers, Merciful and Gracious King, that by the merit and power of lighting the candle that I am about to light, it be as it is written: “The candle of G-d is the soul of man” (Proverbs 20:27), “For a mitzvah is a candle, and Torah is light” (Proverbs 6:23). Illuminate the soul of (name and mother's name), and allow me to bind myself to Rabbi David Zakut (zikhrono livrakha), and may my intention correspond to his intention and to the understanding of all the tzadikim and chasidim. Allow me to light a candle for the merit and healing of (name and mother's name), so that he may receive a complete recovery, healing of the soul and healing of the body. Through the power and merit of lighting G-d's candle, may the soul of man shine, and may the soul of (name and mother's name) be strengthened and shine in every limb of his body (for men: 248 limbs and 365 sinews), and shine in the supernal lights unto Ein Sof, blessed be He. May he arise from his illness healthy and whole, blessed with long days and years, and may his wellsprings flow outward.)

- “Shav'ateinu Kabel Ushma Tza'akatenu Yode'a Ta'alumot”—(receive our plea and hear our cry, You before Whom the hidden is revealed);
3. Tehillim 90:17 [recite 7 times]וִיהִ֤י ׀ נֹ֤עַם אֲדֹנָ֥י אֱלֹהֵ֗ינוּ עָ֫לֵ֥ינוּ וּמַעֲשֵׂ֣ה יָ֭דֵינוּ כּוֹנְנָ֥ה עָלֵ֑ינוּ וּֽמַעֲשֵׂ֥ה יָ֝דֵ֗ינוּ כּוֹנְנֵֽהוּ׃
- “Vihi noam Adonai Eloheinu aleinu, u-ma'aseh yadeinu konenah aleinu, u-ma'aseh yadeinu konenehu”—
(May the pleasantness of the L-rd our G-d be upon us; establish the work of our hands for us—yes, establish the work of our hands);
יהי לרצון אמרי פי והגיון ליבי לפניך יהוה צורי וגואלי
- “Yihyu le-ratzon imrei fi, ve-hegyon libi, lefanekha Ado-nai tzuri ve-go'ali”—(May the words of my mouth and the meditation of my heart be acceptable before You, O L-rd, my Rock and my Redeemer.)
- “Ve-nizkeh meherah kulanu yachad le-kabel be-ezrat Ha-Shem penei Mashiach Tzidkeinu ba-karov mamash be-Eretz Yisrael”—(And soon may we all merit to welcome our righteous Mashiach, with G-d's help, right here in the Land of Israel).

Immediately after lighting the candle, Tehillim 139 should be recited for healing.

- Atah yadati shivti ve-kumi, banta le-rei'i me-rachok.
- Orchi ve-riv'i zerita, ve-khol derakhai hiskanta.

- Achor va-kedem tzartani, va-tashet alai kapekha.
- Peli'ah da'at mimeni, nisgeva lo ukhal lah.
