U-ve-koach shnei Shemot ha-eilu, Adonai Shaddai, ani gozer (gozeret) alecha — ha-sar ha-gadol ve-ha-nora Mikha’el — she-temaher le-havi tefilati mi-sha‘ar le-sha‘ar u-mi-makom le-makom, bimkomot ha-rachamim ve-ha-heichalot, ad she-tavi otah ve-ta‘aleh le-ma‘alah lifnei Tzur Me‘on, ve-taktirenah ke-ketoret ha-samim. Ve-alecha, Mikha’el, ha-massa ha-zeh la‘asot, ki Shemot nora’im zacharti alecha; ve-lo le-hachrichacha zichartim, ela she-temaher ha-davar la‘asot mipnei kedushatam u-gevuratam. Ve-hineni, Adonai E-l Rachum ve-Chanun, Erech Apayim ve-Rav Chesed ve-Emet, kore’a (korah) u-mishtachaveh (u-mishtachavah) lefanecha, ve-sho’el (ve-sho’elet) mimecha be-lev shalem, nishbar nidkeh, ve-ruach shefelah, ve-kelayotai mit’haradim mipnei rov gevurotecha, she-ta‘aseh she’elati ve-tashlim bakashati, she-hi kach ve-kach [say your requests here]. Ki Atah Shome’a tefilot u-mekablam, Elo-hai kol ha-aretz, le-avadav ha-kor’im bi-Shemecha. U-kemo she-Atah Shome’a tefilot, ken Atah shema be-rov rachamecha tefilati ve-aseh bakashati, Eh-yeh Asher Eh-yeh, Adon ve-Melech al kol ha-aretz. Baruch Atah, Adonai, Shome’a tefilah.

On Shabbat, it is preferable to make the request as it is written in Birkat Shme: “May it be Your will to grant us long life and well-being, so that we may merit Your favor, and that You guard me and all that is mine, for You provide sustenance for everyone and grant each person everything necessary.”

Translation:

And from You, O my G-d, the sixth light spreads forth and emerges through its boundary, turning downward according to its statute and law, to influence the creations below and all the hosts of the earth. It stands between two paths, and all things reach it first. It alludes to Knesset Yisrael and is called Adonai Shaddai, from the places of prayer. By the power of the two Names — Adonai Shaddai — I invoke you, camps of the Aralim, who are drawn forth with mercy, to hasten to fulfill this request of mine: [say your requests here]. By the power of these two Names — Adonai Shaddai — I command you, Mikha’el, the great and awesome prince, to hasten to carry my prayer from gate to gate and from place to place, through the places and chambers of mercy, until you bring it and raise it on high before Tzur Me‘ono, and offer it like incense. And upon you, Mikha’el, rests this task: to carry it out, for I have invoked awesome Names upon you. I do not mention them to compel you, but so that you will hasten to perform this deed because of their holiness and power. Here I am, Adonai, the merciful and gracious G-d, slow to anger, abundant in kindness and truth. I kneel and bow before You, asking You with a whole, broken, and contrite heart and a humble spirit, with all my inner being trembling before Your great might: fulfill my request and grant my plea, which is as follows: [say your requests here].

For You hear prayers and accept them, O G-d of the entire earth, for Your servants who call upon Your Name. Just as You hear prayers, so too, in Your abundant mercy, hear my prayer and fulfill my request, Eh-yeh Asher Eh-yeh, Master and King over the entire earth. Blessed are You, O L-rd, who hears prayer.
